The World Health Organization approved the R21 malaria vaccine for widespread distribution across 18 sub-Saharan African countries on September 1, following clinical trial results showing 77 percent efficacy in children aged 5 to 17 months. The vaccine, developed by Oxford University and manufactured by Serum Institute of India, will be available at a cost of $2 per dose, making it the most affordable malaria vaccine in history. WHO Director-General Tedros Adhanom Ghebreyesus called the approval "a historic moment that could save tens of thousands of lives annually." "Malaria has killed more people than any other infectious disease in human history," Tedros said during a press conference in Geneva. "This vaccine gives us a powerful new tool to turn the tide." ## Clinical Trial Results The Phase III clinical trial, conducted across Burkina Faso, Mali, Tanzania, and Kenya, enrolled 4,800 children and demonstrated 77 percent efficacy against clinical malaria over a 12-month follow-up period. The trial, published in The Lancet in July 2026, showed that the vaccine reduced severe malaria cases by 68 percent and hospitalizations by 54 percent. "The R21 vaccine represents a significant improvement over the RTS,S vaccine approved in 2021," said Adrian Hill, director of the Jenner Institute at Oxford University. "The higher efficacy, lower cost, and scalable manufacturing make this a game-changer for malaria control efforts worldwide." Malaria killed approximately 608,000 people in 2024, according to the WHO World Malaria Report, with 95 percent of deaths occurring in sub-Saharan Africa. Children under five accounted for 80 percent of fatalities across the region. ## Manufacturing and Distribution Serum Institute of India has committed to producing 100 million doses annually, with the first batch of 25 million doses expected to ship in Q1 2027. The company invested $300 million in a dedicated malaria vaccine production facility in Pune. "We have the manufacturing capacity to meet global demand within two years," said Adar Poonawalla, CEO of Serum Institute. "The challenge is not production. It is cold chain logistics and healthcare delivery in rural communities." Gavi, the Vaccine Alliance, has allocated $1.2 billion to support vaccine procurement and delivery across eligible countries. The program will integrate malaria vaccination with existing routine immunization schedules, targeting children at 5, 6, and 17 months of age. ## Economic and Social Impact The economic burden of malaria in sub-Saharan Africa exceeds $12 billion annually in lost productivity, healthcare costs, and premature deaths, according to the WHO. The vaccine is expected to generate a return of $36 for every $1 invested when accounting for reduced healthcare utilization and improved educational outcomes. School absenteeism due to malaria episodes costs the region an estimated 2.7 million school-days annually. Health economists project that the vaccine could recover 1.4 million school-days per year within three years of widespread deployment. ## Eradication Campaign Momentum The R21 approval builds on progress from the RTS,S vaccine, which has been administered to over 2 million children in Ghana, Kenya, and Malawi since 2022. Combined with insecticide-treated bed nets, seasonal malaria chemoprevention, and improved diagnostics, the new vaccine could reduce malaria mortality by 50 percent by 2030. "We are not just adding a tool. We are changing the trajectory of the oldest pandemic in human history," said Pedro Alonso, director of the WHO Global Malaria Programme.
